Considerations for successful Foam Sclerotherapy
1 Pre-operative duplex ultrasound examination of the leg is mandatory
2 Use Terumo syringes and Vygon 3-way tap with Luer Lock connections to make stable foam using the Tessari technique
3 Use one part liquid sclerosant to three or four parts of air
4 Prepare foam immediately before use
5 Administer foam in no greater than 2ml aliquots approximately 10-15 cm apart in the truncal veins and use a maximum of 16ml of foam per patient per 24hours
6 Empty the vein of blood as much as possible before foam injection, for example by raising the leg in the air or flushing with saline
7 Use 3% Fibrovein foam in truncal veins below the fascia and use 1% Fibrovein foam in more superficial veins
8 Remember sodium tetradecyl sulfate is at least twice as potent as polidocanol
9 Apply eccentric compression following foam sclerotherapy 10 Encourage patients to walk frequently after foam sclerotherapy

Considerations for successful Microsclerotherapy
1 Perform an ultrasound examination of the limb to exclude underlying venous insufficiency
2 Use liquid sclerosant—there is no benefit to using foam in small diameter veins
3 Use 0.5% Fibrovein for treating reticular veins and 0.2% Fibrovein for treating spider veins
4 Consider further dilution of 0.2% Fibrovein to 0.1% with normal saline for very fine diameter veins1,2
5 Use fine gauge needles typically 30G
6 Treat an area of no greater than 2 cm diameter with each injection
7 Inject no more than 0.1– 0.2ml volume with each injection
8 Use a maximum of 10ml 0.2% or 0.5% per patient per 24hours
9 Remember sodium tetradecyl sulfate is at least twice as potent as polidocanol, so use 0.2% for spider veins 10 Apply concentric compression followin

Microwave ablation (MWA) is a minimally invasive procedure that uses microwave energy to heat and destroy abnormal tissue, such as tumors. A small probe or antenna is inserted into the target area under image guidance (like ultrasound or CT), and microwave energy is emitted to raise the tissue's temperature to over 100°C100 degrees cap C100°, killing the cells. It can be used to treat various tumors, including in the liver, and has advantages like faster heating, larger treatment areas, and the ability to ablate multiple tumors at once compared to older methods like radiofrequency ablation.

Applications
  • Liver cancer: Commonly used to treat early-stage liver cancers.
  • Breast tumors: Used for small, benign breast tumors (fibroadenomas).
  • Other cancers: Can be used for other cancers where similar ablation techniques are used, such as in the lungs.
Advantages
  • Faster and more efficient heating: Microwave energy heats tissue faster and more uniformly than other methods.
  • Larger treatment area: Can ablate larger tumors more effectively.
  • Less susceptible to the "heat sink" effect: The high heat generated is less affected by blood vessels that would normally cool the tissue, allowing for more complete destruction in some cases.
  • Simultaneous treatment: Can be used to treat multiple tumors at once.
